Two paver sizes are available – 12″ x 12″ = $250.00 (304 available); 24″ x 24″ – $5,000.00 (36 available).
To order a paver, download the form below, or pick one up at the Parish Office, and complete. Bring the order form and payment in full to Mass and present at the kiosk in the lobby of the Chapel or during the week at the Parish Office. Three forms of payment will be accepted: credit card, check or cash.
